Social Worker – Transtions

37 hours Per week permanent Post

Monday – Thursday 8.30 – 5.00, Friday 8.30 – 4.30.

If you are positive about enabling young people to move into adult services in a person-centred way and have a creative approach to support planning, then we want to hear from you.

The Transitions Service support young people from the ages of 16-25 with severe and enduring disabilities to move into adult services in a planned and personalised way.

We have a great opportunity for a motivated and proactive Social Worker who is committed to ensuring that the service supports young people and their families to make choices about their future that are individual, timely and appropriate to join our established and skilled team.

To fully support young people, the service sits within Adult Social Care. However, our work necessitates close working partnerships with Childr07788 356 471en’s Disability Social Care, Education and Health Services. We work within the Legislative Acts and Frameworks covering both children and adults. Knowledge, experience, and an understanding in this area of social work would be advantageous. You will need to have experience of working with young people with disabilities and their families.

It’s a challenging role - but there’s no shortage of rewards either – from being part of a cohesive, friendly and supportive team to making a real difference to the quality of life of the people we work alongside, you will be part of a large Council with many development opportunities.

There is likely to be the expectation of working from home in the Covid 19 recovery period. However, you will need to be able to independently travel to cover the catchment area of the team and to fulfil the needs of the role.

Your starting salary will be pro rata if the above position is part-time or term-time only.
