Salary Range: Band 4 NHS Agenda for Change terms and conditions - £21, 892 - £24,157pa pro rata
Hours: 28 hours per week

We have a Healthcare Assistant post in our Adult Disability Service working with adults with Learning Disabilities from 18 years. As part of the Community Learning Disability Nursing service you will be supporting health providers like GP’s to make reasonable adjustments in providing high quality care for people with learning disabilities.
You will be working with nursing and social care colleagues to ensure that people with learning disabilities living in the community are supported to manage their own health needs as independently as possible. You will be working collaboratively with carers and GP’s to ensure that proactive health interventions minimise the risk of health inequalities for this population. In your supporting statement please demonstrate how you have the skills and qualities required within the Job Profile. You will also need to be able to drive as you will be required to travel across the county at times.
