An exciting opportunity has arisen to be part the social prescribing team within Public Health, supporting Medway’s older population to become less socially isolated.
The aim of this role is to promote social inclusion by helping service users identify and achieve their goals. The postholder will be required to meet with people referred into the service on a one-to-one basis carrying out home visits where appropriate. You will provide non-judgmental support, working from a strength-based approach and focusing on a person’s assets; giving time for service users to focus on ‘what matters to me’. Where appropriate, you will physically introduce people to community groups, activities and statutory services, ensuring they are able to engage, happy and included.
You will manage your own caseloads in accordance with the needs, priorities and any urgent support required by the service users. You’ll need to have the sensitivity and understanding to be able to work with vulnerable adults, with a strong awareness of when what the person needs is beyond the scope of the link worker role and it is appropriate or necessary to refer people to other health professionals/agencies.
Successful candidates must:
- Be passionate and committed to improving the lives of Medway’s socially isolated older population
- Be compassionate, resilient and organised
- Have excellent listening, communication and interpersonal skills
- Have experience of supporting people through behaviour change, in a group or one to one setting using motivational interviewing and CBT principles
- Represent Public Health and champion the Social Prescribing agenda
- Be able to work independently and make decisions without access to a manager
- Be a strong team player with the flexibility to cover colleagues on occasions such as annual leave or sickness absence.
- Hold a full driving license – non negotiable
