Job Title: Social Worker – East and North Herts Older People’s Service, Integrated Hospital Discharge Service
Location: Primary base - Farnham House, Stevenage
Salary Range: H8-M1£27,741 - £39,880pa plus £6,000 relocation package, £1000 welcome payment & lease car* terms apply
Hours: 37 hours per week
An exciting opportunity has arisen in our Post Hospital review Team Our Service supports an integrated approach to managing the care of people who require support upon discharge from hospital, so the role will involve working alongside occupational therapists, community care officers, health colleagues and care providers.

You will be required to review support packages of people discharged from hospital on our various enablement pathways, including carrying out assessment of need and setting up longer-term packages for those requiring on-going support. You will also need to undertake safeguarding activity if required.
We are a busy team covering a large and diverse area and welcome applications from candidates who can bring energy, creative care planning when reviewing care in place following hospital discharge and promote citizen focussed assessments and reviews.
You will need to be SWE registered and have knowledge and experience of relevant legislation. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.
We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enable people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.
You should be able to demonstrate a commitment to supporting people with the right care and information at the right time to help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to.
The post will require you to visit service users in the community to undertake reviews, so a driving licence would be advantageous as is the ability to respond to situations quickly and flexibly.
To improve outcomes for service users, the Integrated Discharge Service supports a 7 day Hospital Discharge service. The post will be asked to be part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after.
