Two positions available:

  • Health & Social Care Integration Project Manager - Permanent
  • Voluntary Sector Integration Project Manager – Fixed Term until 31st March 2022

The Wokingham Integrated Partnership, is a mature and respected partnership, including the council, Voluntary and Community Sector, Community Nursing, the Clinical Commissioning group, Public Health and more.  As a project manager, you will be supporting the ongoing integration and joint working of the partnership to get the best outcomes for the people of Wokingham. 

The Health and Social Care Integration Team are currently leading the integration vanguard in many areas, with our Population Health Management work being shared nationally by NHS England as best practise and sharing this learning with the World Health Organisation. 

As a member of the Health and Social Care integration Team, you will be required to draw upon your knowledge of project management, integration of health and social care and voluntary sector stakeholder management, to lead integration projects for the Wokingham Integrated Partnership and the Wokingham Wellbeing board. 

You will plan, implement and deliver major, complex and voluntary sector transformation projects in accordance with approved project management methodologies.  This role contributes to the operation of the Integration Programme Office function, operating within Corporate project and programme management processes and requirements. 

To fulfil this role, you will communicate well and work collaboratively with team members & Stakeholders.  With your knowledge of transformation/Integration of Health & Social Care agenda, you will have a proven ability in managing projects, other managers, and staff to achieve agreed outputs, within budget, including benefit realisation.
