Job Title: LeDeR Health inequalities Nurse
Location: Farnham House, Stevenage/Apsley, Hemel Hempstead
Salary Range: Band 6 - £31,365 to £37,890 pa (pro rata for part-time)
Hours: 37.5 hours per week but part time will be considered

Are you a Qualified nurse with experience working with people with learning disabilities? We are looking for 2 Community nurses to support us to reduce health inequalities by working alongside primary care colleagues to;
- Improve the number and quality of Annual Health Checks carried out each year
- Improve the uptake of cancer screening programmes through ensuring a positive experience when first invited for mammograms/cervical smears and bowel screening checks
- Improve the uptake of flu vaccinations each year
- Work closely with Primary Care Networks to ensure targeted areas of health improvement reflect the needs of people with learning disabilities
You must have an active nursing registration and experience in working with people with learning disabilities and being a car driver and access to a vehicle is essential due to the nature of the role. Experience working in primary care with an understanding of the ways that PCNs function would be beneficial. You will be working Mon -Fri 9 -5 alongside community learning disability nursing colleagues to achieve clear targets from commissioners in reducing health inequalities through proactive interventions. You will have access to a wealth of training opportunities and support from Senior Nurses in this new role, along with all the benefits accessible to HCC employees.
