About the role
We are able to offer an exciting opportunity for one full-time Senior Community Support Worker to join our existing team working within our Dementia Support Service.
Working within the Dementia Support Service you will be part of a supportive team promoting safety, inclusion and personalization to People living with Dementia and their Carers.
Our Dementia Post Diagnostic Services offers long term support to people living with Dementia, their families and Carers. You will be expected to visit adults and carers in their own home and work with them to promote choice, safety and independence to live well with Dementia and encourage them to make informed decisions for their future needs. Your role will include support, planning and collaborative working with other agencies.
Part of this role may involve supporting clients through the memory assessment process, supporting carers and completing carers' assessments, and may involve supporting Cared for adults to attend a community activity as a one off taster session; this will involve transporting adults in your own vehicle.
This role involves working countywide. A clean driving licence is a requirement. Business mileage will be paid.
You will be required to offer support and guidance to the team of Support Workers.

Hours of work: 37 hours per week (Monday - Thursday: 9am to 5pm and Friday 9am to 4.30pm)
Weekend work is not required but some flexibility may be needed.

Your starting salary will be pro rata if the above position is part-time or term-time only.
