You will be working with young people aged between 16 and 25 undertaking needs assessments and creating Pathway Plans with them to support their needs so that objectives and aspirations are met.
You will be able to effectively manage a caseload to ensure that all departmental and statutory procedures and requirements are met. You will maintain accurate and appropriate records on all work undertaken in accordance with the departmental procedures. As part of the Leaving Care Team you will participate in the team duty rota so that enquiries can be dealt with promptly. The ideal candidate will hold an NVQ 3 in Child Care or sufficient knowledge to achieve this qualification within 6 months.
