Hertfordshire County Council are looking for an experienced Community care officer, who will work to reduce social care admissions through the Lister Hospital Emergency Department.
The team’s professionals work in a 7-day, 8am-8pm Emergency Department Social Work team with close links to Clinical Navigators, Integrated Discharge Team and Emergency Department health staff. The post holder’s work will include the assessment of A&E attendees to reduce unnecessary admissions and reduce acute length of stay.
You will need to be a self-starter who enjoys a challenge and works effectively with a wide range of professionals and across organisational boundaries.
You will be someone that has some experience of working within in a caring and supporting environment. You will be someone who is able to demonstrate that they can work independently and have good communication skills.
You will be someone who is computer literate and used to maintaining electronic records.
