Job Title: Advanced Practitioner
Location: Primary base – Farnham house (but will be able to work remotely)
Salary Range: M1-M2 £35, 745 - £43,857pa plus £1,000 welcome payment
Hours: 37 hours per week

Our team’s purpose is to support older people to maintain their independence and wherever possible, to stay in their own homes. You will be a registered professional with Social Work England. You will have sound understanding of relevant social care legislation and a commitment to the person centred and relationship-based practice that is the hallmark of our team’s approach.
You will lead on practice. We’ll look to you to inspire, develop, nurture and support the way we work with people. You will contribute on our behalf to the ACS Advanced Practitioner’s Forum and ensure that our practice continues to develop in line with our Connected Lives practice model as well as national and local practice themes and developments.
You will hold regular practice peer groups, ensure good links to ACS Practice Development opportunities and provide mentoring and supervision for practice colleagues.

We need you to be effective, efficient and to bring creative ideas about how you will support our quality and performance. You’ll work closely with our Team Manager and Deputy Team Manager and will be on hand to offer reflective support and guidance for colleagues as they encounter the complexities of practice.
