Job Title: Rapid Response Social Worker
Location: Apsley, Hemel Hempstead
Salary Range: H9-M1 £32,234 - £39,880pa plus £1,000 welcome payment
Hours: 37 hours per week
Team: Three Rivers Older Persons Team
Adult Care Services is looking to recruit a highly motivated and experienced SWE registered Social Worker to join the Three Rivers Older Persons team as the lead practitioner for Rapid Response. You must have completed your ASYE. This is an exciting opportunity to work closely with both Social Care colleagues and a developing multi-agency, integrated service combining Health, Social Care and Mental Health services. The aim of this role is to reduce unnecessary hospital admissions/carer breakdown and to promote independence, by assessing people in their own homes and assisting service users to maximise independent living.
You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being. You will keep the service user at the centre of all activity and address assessed needs within the legislative framework. A good understanding of the relevant adult social care legislation is essential to this post. You will need to be able to work autonomously within agreed timeframes and manage frequently changing circumstances. You will work as part of an integrated team, attend multi-disciplinary meetings and need to have excellent communication skills.
For this role you need to have a recognised qualification in Social Work and be at least 2 years post qualification and be registered with SWE. The successful applicant will need to demonstrate sound knowledge and experience of safeguarding practice and skills in risk assessment and positive risk taking which draws on a service user’s strengths and assets.
