Experienced Social Worker- Warrington Integrated Discharge Team
Halton Borough Council is offering a unique opportunity for an Experienced Social Worker (HBC 8) to become part of the Hospital Integrated Discharge Team (based in Warrington Hospital).
Halton Borough Council’s expectations are high and you will need a strong commitment to providing first class person centred care to Halton residents over 18 and their Carers.
This Social work role involves Discharge planning. Applicants need relevant experience in completing assessments and planning to facilitate safe and timely discharges from hospital. You will be able to manage and organise your own case-load.
Relevant working knowledge of social work legislation such as the Care Act, Capacity Act and a good understanding of the principles is essential for this role as well as being able to complete risk assessment and management. Applications are invited from Social Workers who have achieved AYSE and submission of a successful ‘Development Portfolio’
You will be passionate about working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties), whilst challenging discrimination and promoting independence.
We are looking for an enthusiastic and committed team player and be dedicated to Multi agency team working within the hospital setting. During these uncertain times of Covid 19, the successful applicant will at times need to visit wards to support timely discharges. All PPE will be available and safe working systems will be in place.
You will have a strong personal commitment to anti-oppressive practice and be able to demonstrate your commitment to challenge discrimination through your daily practice. Applicants must have good interpersonal and communication skills, a flexible approach and a willingness to learn new skills and continually develop. Ideally you will have good IT skills.
You will receive a car user allowance and a flexible working week. You will receive up to 33 days annual leave per annum and a commitment to your on-going professional development.
