An opportunity has arisen for an Advanced Social Worker to join a busy multi-disciplinary, multi-organisational, integrated hospital discharge team in Warrington Hospital, on a full time, permanent basis. Within the role you will be responsible for rapid assessments of needs in preparation for service users to be discharged home or to a longer term placements, in timely and safe manner.
The acute hospital trusts are responsible for managing bed capacity and demand on a daily/hourly basis, requiring all partner organisations to work together to ensure flow through the acute hospital setting is efficient, effective, flexible and responsive. We are looking for a confident, organised practitioner, with a good working knowledge of the complexities of Hospital discharges planning.
Within this role you must be commited to working within the framework of legislation, agreed policies and procedures of work with, or on behalf of, individuals and their families. This role will support social care and health staff, providing guidance and professional wisdom, demonstrating expert and effective practice in complex situations; assessing and managing higher levels of risk and be responsible for the day to day work allocation within the team in the absence of the Practice Manager.
During these uncertain times of Covid 19, the successful applicant will at times need to visit wards to support timely discharges. All PPE will be available and and safe working systems will be in place.
This is a great opportunity to put your Social Work England registration and Social Work qualifications to great use. You will have practical experience of working with service users and will support colleagues by developing and providing training resource for Social Workers, Support Staff and Carers.
