We currently have a vacancy for a Social Care Assistant within our Adult social care team at Epsom & St Helier General Hospital.
We are looking for enthusiastic applicants who enjoy working with people and can work in a high pressure, acute hospital environment. Successful candidates will have an important role in the team, helping to deliver high quality personalised adult social care and supporting timely, safe and effective hospital discharges and is able to manage the demand to ensure service needs are met.
Promoting choice and control is at the heart of everything we do. Your role will involve a variety of tasks including carrying out person-centred assessments and support planning, risk assessment and reablement for some of Surrey’s most vulnerable residents. You will frequently be the first point of contact between the general public and the service, gathering initial information and identifying the need for further assessment, as well as signposting people to other services.
On a day to day basis you will be part of a team and have access to direction, support and supervision. You will have your own caseload and work with people who need additional support or guidance in order to identify their needs or where best interest decisions need to be made on their behalf. You will be involved in supporting safeguarding section 42 enquiries with the support of qualified practitioners.
You will work in partnership with individuals, carers, families, professional health staff, other agencies, service providers and support brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.
The role will include doing duty work, attending ward Multi-disciplinary meetings and family meetings.
Applicants will enjoy working with people and have experience of direct work with the general public as part of paid or voluntary work.
You will need to have competent IT skills and be able and willing to learn to use a variety of software systems.
Please note as this role requires you to travel around the county, you will need to demonstrate an ability and willingness to travel around Surrey. Therefore you need to have a full driving licence and have access to a car.

Our service runs from 8am to 8pm Monday to Friday and 9am to 5pm Saturday and Sunday. In practice you are likely to be required to work a minimum of one weekend shift per month and one late shift per week. Staff who are on the rota to work a shift falling on a weekend will be given a day off in lieu. Full time applicants will be contracted to a 36 hour week and these hours will be worked on a rota basis across 7 days a week.
