Our service runs from 8am to 8pm Monday to Friday and 9am to 5pm Saturday and Sunday. In practice you are likely to be required to work a minimum of one weekend shift per month and one late shift per week. Staff who are on the rota to work a shift falling on a weekend will be given a day off in lieu. Full time applicants will be contracted to a 36 hour week and these hours will be worked on a rota basis across 7 days a week. Exciting new changes are happening at St Peters Hospital Social Care Team in Chertsey to improve patient experience through timely discharges. As a result we have enhanced our team which has resulted in us being able to recruit to new posts. There has never been a more exciting time to join Adult Social Care, we have had a major review of how we work and are currently implementing these changes.
Our role is to Discharge to Assess residents of Surrey in a safe and timely manner. You will be working closely within a multi disciplinary team and will always be working within a whole system approach. You will be an excellent communicator, motivated, supportive, flexible, creative and a team player.
You will need to be confident in undertaking safeguarding enquiries, carryout assessments within the hospital and also the community. You will be able to communicate in a clear, fluent, concise and jargon free way and in a courteous, calm and professional manner. You will be expected to have a sound knowledge and experience of current legislation, including the Care Act, Continuing Health Care and the Mental Capacity Act and its application to practice in a social care setting.
You will be required to supervise less experienced members of staff within the team and support their development. In return for all of your skills, enthusiasm and commitment we will support you fully in becoming the social worker you want to be.
