Integrated Hospital Discharge Service
This is an exciting opportunity in a newly established team within our Integrated Discharge Service. Based in hospitals across the County, this Service supports an integrated approach to managing care of people who require support on discharge from hospital. The Post Hospital Review Team work closely with external partners to undertake social care reviews of people discharged from hospital on our various enablement pathways.
The successful applicant will reviewing arranged packages of care that have been arranged to facilitate discharge from the acute hospitals to their own homes. You will keep the user and any future potential users at the centre of all activity and address user needs within the legislative framework.
We are a busy team covering a large and diverse area and welcome applications from candidates who can bring energy, creative care planning when reviewing care in place following hospital discharge and promote citizen focussed assessments and reviews.
To improve outcomes for service users, the Integrated Discharge Service supports a 7 day Hospital Discharge service. The post will be asked to be part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends. There will be opportunities for you to discuss this at interview if your application is successful.
