Location: Aintree Hospital
Contract: Fixed term until 31st March 2022
Full Time - 36 hours per week.

An exciting opportunity has arisen to join the Adult Social Care Service within Sefton Council on a fixed term contract basis. We are seeking applicants with backgrounds in Hospital and/or community Social Work.
As a qualified social worker, you will contribute to the delivery of an effective Adult Social Care service as part of a team responsible for facilitating safe and timely discharges from Hospital for Sefton residents.
You will manage a defined caseload delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual packages of care, step down beds and care home placements whilst managing the reputation of the Council.

Due to the nature of work the above post is exempt from the provisions of the Rehabilitation of Offenders Act 1974 and / or relevant care / safeguarding legislation and will only be appointed if a satisfactory Enhanced Disclosure is obtained from the Criminal Records Bureau.
