The first vacancy is within our Transition Service supporting young people from the ages of 16-25 with disabilities to move into adult services in a planned and personalised way. We are looking for a motivated and proactive qualified Social Worker who is committed to ensuring that the service supports young people and their families to make choices about their future that are individual, timely and appropriate to join our growing team.

If you are positive about enabling young people to move into adult services in a person centred way and have a creative approach to support planning then we want to hear from you.

You will need to have recent experience of children’s social work and a thorough knowledge of relevant legislation relating to children with disabilities.

The second vacancy is within our Adult Learning Disability Assessment and Care Management Team - West, based at The Bellbrook Centre, Uckfield.

The role involves supporting adults with a learning disability to live successfully within the community, helping them maximise their strengths and supporting through the provision of a care management service to both service users and carers. Self-Directed Support is a key feature of the role as is the ability to act as a Lead Enquiry Officer (LEO) using safeguarding case management.
Positive outcomes for clients are our top priority. But we know that to achieve this, you need the support and professional development to make the most of your skills. That’s why at East Sussex, you’ll find manageable workloads but with a broad range of challenges, excellent training and regular development opportunities. That’s our commitment to you.
