We helped June into a new home. Now she’s got a new lease of life.
We want to develop and improve the experience of the people we support to enable them to live as independently as possible and our adult community and intermediate care service has a vital role to play in that. Through continual learning and reflection of our practice we can support our older people in their daily lives and give them the time and support they need to make an impact on their lives. One of the ways we are doing this is by building an exciting new care home which will help people who need extra support following discharge from hospital and for people with Dementia. This is an opportunity to take ownership. And to change lives for the better.
Because we’re Bracknell Forest.
