Health Care Assistant in 0-25 Together central team working across East and North Hertfordshire.

We have a Healthcare Assistant post in our 0-25 Together Service working with adults with Learning Disabilities from 18 -25years of age. As part of the wider Community Learning Disability Nursing service you will be supporting health and social providers to make reasonable adjustments for youngsters going through the change from children to adult health and education services.
You will be working with young people with learning disabilities and their paid/family carers to ensure that people living in the community are supported to manage their own health needs as independently as possible. This involves supporting young people to learn about several topics such as healthy relationships, managing their feelings and ensuring risks of health inequalities are minimised through education. You will be working collaboratively with health and education partners to ensure this period of change and anxiety is as smooth as possible. You will be supporting the implementation of individual care plans with team colleagues to ensure health needs are met in a person-centred manner working directly with individuals under the direction of a qualified nurse.
