About the role
The Through Care Service works with care experienced young people aged between 14 -25. We are a dynamic, multi-disciplinary team which includes social workers, senior social workers, personal advisors, a mental health nurse and the virtual school. The Through Care Service also includes a team to support and assess Unaccompanied Asylum Seeking Children.
As a social worker in Through Care you will be responsible for supporting young people across the county whilst they prepare to leave care and after they leave. Assisting them in reaching their aspirations. You will have case responsibility for young people up to the age of 18 and will have opportunities to continue to support young people over the age of 18, ensuring that the principles of relationship based practice are maintained.
You will have opportunities to work with a wide range of partner agencies including education, housing, health services and the police in order to ensure the best outcomes possible for our young people.
The role will be varied and can be challenging at times, it will involve assessing and managing risk and the impact of trauma. You will receive the support of a committed and experienced team, regular supervision and a wide range of training opportunities.
There will be a need to travel cross county and sometimes nationwide dependent on where the young people are living.
