An opportunity has arisen for a part time Social Prescriber to work in partnership with our South Blaby and Lutterworth Primary Care Network to deliver the Social Prescribing Service.
It’s an exciting time to join us as we are looking for an enthusiastic and motivated individual who would be working for Countesthorpe, Wycliffe and Masharani GP practices to deliver holistic interventions to improve the health and wellbeing of our local residents.
This is an excellent opportunity to work within the South Blaby and Lutterworth Primary Care Network.
You will need to have strong communication skills to enable you to build and sustain collaborative working relationships; including educating partners on the service and informing them of the social prescribing options that are available across the Districts.
We are also looking for people with some experience of working along people with vulnerabilities who can empower residents in a planned and structured way to improve health, recovery and wellbeing outcomes.

The Main Council Offices are in a lovely little village called Narborough in Leicestershire which is just 3 miles from the M1, junction 21 and not far from the centre of Leicester. The local train station is only a 5 minutes walk away and there are regular bus links to and from the city centre.
