We have a dynamic Transition Team in Adult Social Care and are in need of an Experienced Social Worker. In the team we provide support for 14 to 25 year olds with complex and significant disabilities. Our work is aiding their transition around independence skills, education and work.
We assess for direct payment needs, as well as the provision of supported living. As a team we work closely with Special Educational Needs and Disabilities (SEND) and aid Education Health Care Plan requirements and Continuing Health Care support, assisting around transition to adulthood needs and at times the necessity for residential school.
We as a team complete Self Directed Assessments, Mental Capacity Assessments, Best Interest Assessor work, Reviews and Carers Assessments. Our role also involves supporting Children in Care to Transition to Adulthood, as well as Special Educational Needs and Disabilities (SEND) to support Adult Social Care requests.
