Job Title: Social Worker, Hertsmere Extended Involvement Team
Salary: £27,741 - £39,880 + £6,000 relocation package, £1000 welcome payment & lease car* terms apply
Hours: 37 hours per week
Adult Care Services is looking to recruit a highly motivated and experienced SWE registered Social Worker join the Hertsmere Older Persons Locality Team. Your role will primarily be to carryout comprehensive assessments to enable support and assist those in need of social care support to achieve their identified outcomes. You will promote independence and enablement for elderly people who are struggling to manage activities of daily living in their homes or in placement.
You will be working to respond preventatively to referrals for social care and support and will use social work interventions and a strengths-based approach to enable adults and older adults to remain in their own homes where’s possible with the optimum level of independence.
You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being. You will keep the service user at the centre of all activity and address assessed needs within the legislative framework. A good understanding of the relevant adult social care legislation is essential to this post. You will need to be able to work autonomously within agreed timeframes and manage frequently changing circumstances. You will work as part of an integrated team, attend multi-disciplinary meetings and need to have excellent communication skills.
As part of your role in Hertsmere there will be an opportunity to work closely with Community Health colleagues, Mental Health services, the District Council and other universal and voluntary services.
For this role you need to have a recognised qualification in Social Work and be registered with the HCPC or Social Work England. The successful applicant will need to demonstrate sound knowledge and experience of safeguarding practice and skills in risk assessment and positive risk taking which draws on a service user’s strengths and assets.
