Hours: 37 hours per week

You will source and match care provisions for Adults of North Somerset regardless of who funds their care. These people may have extremely complex/challenging needs and therefore you will need to be aware of the services in North Somerset that can support people in their own homes and in a Care Home setting.
You could be working in a variety of settings for example in the community, office based and with various hospitals. The ideal candidate would be passionate about supporting people to remain as independent as possible using various services to support them. You will need to be organised, enthusiastic with the ability to problem solve, prioritise and manage workload using your own initiative.
This post is exempt under the provisions of the Rehabilitations of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.
