Social Care Assistant position to work flexibly between all hospitals in South West Surrey therefore applicants must hold a valid driving licence and have access to a car.

Following changes within the South West area of Surrey, Royal Surrey County Hospital have taken over management of the social care function within the three community hospitals in the area: Milford, Haslemere and Farnham.

As such, we have an exciting opportunity where we are looking for staff to work flexibly between the Royal Surrey County Hospital in Guildford and the three community hospitals. You will receive excellent support and supervision combined with comprehensive training/induction.

Your role will involve a variety of tasks including assessing patient’s needs and working closely with the multi-disciplinary team.

Our aim is to facilitate patient flow by supporting discharges to the destination of choice in a timely manner for social care and privately funded patients by assessments on the ward and liaising with care providers.

You will also be involved in supporting safeguarding investigations and attending safeguarding meetings with the support of qualified practitioners.

As a Social Care Assistant, you will have some experience of working directly with people with social care needs. You will be confident and competent in working with people with a variety of physical and cognitive impairments.

You will need to have competent basic IT skills and be able and willing to learn to use a variety of software systems.

The work involves travelling to other hospitals within South West Surrey therefore you will need to be able to be able to travel to other bases if required.

You also need to communicate in a clear, fluent, concise and jargon-free way and in a courteous, calm and professional manner.

You will be working in a team of motivated, dedicated and friendly colleagues and have the bonus of working closely alongside a wider multi-disciplinary team in a thriving hospital environment.
