The Portsmouth Adult Mental Health Recovery Team is an integrated health and social care team, providing support for people with severe and enduring mental illness. The team consists of Occupational Therapists, Mental Health Nurses, Social Workers, Psychologists and Psychiatrists.  The post holder will provide skilled support to maximise independence and recovery in the community.

This role is part of the s75 agreement.

 

Your New Role

You will be ensuring that those eligible for social care assistance have the maximum choice, control and power over the support services they receive in order to achieve the best possible quality of life. You will work within the Adult Social Care eligibility criteria to provide assessment and advice to clients and carers, to manage practical tasks through a problem solving approach and to provide creative solutions which meet peoples’ needs and reduce risk. As part of this role, you will assist with self-directed support and engage in telephone and face-to-face visits. You will also be involved in assessing the abilities of clients in all daily living tasks, referring cases to an appropriate professional when needed.

 

What you’ll need to succeed

For this role you will need to demonstrate your passion for working with people with a mental health illness, along with experience working in a care related field. You will need to have the ability to communicate sensitively to a wide range of people, as well as be able to write clear and concise reports and letters. You will need to be approachable to vulnerable people and able to acknowledge and respect individual differences. You will be able to manage your workload effectively whilst remaining calm under pressure.
