Halton Borough Council is offering a unique opportunity for an experienced Social Worker to become part of the team within the Hospital Integrated Discharge Team based in Warrington Hospital.
This Social work role involves hospital discharge planning to facilitate safe and timely discharges from hospital. You will be providing first class support to Halton residents over 18 and their Carers to support their discharge from hospital. You will be passionate about working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties). Assessments will need to be timely, strengths based and person centered. Organisational skills and resilience are required when working in the fast paced MDT arena.
You will be a qualified Social Worker registered with Social Work England, with experience of hospital discharge planning. Relevant working knowledge of social work legislation such as the Care Act, Capacity Act and a good understanding of the principles is essential as you will be ensuring that care and support is provided in line with this legislation.
As a Social worker, you will have a strong personal commitment to anti-oppressive practice and be able to challenge discrimination through your daily practice. You will also have excellent interpersonal and communication skills. This role will provide you with opportunity to work as a team with your Social work colleagues as well as with a variety of hospital professionals, using your IT skills to write reports and attending multi-disciplinary meetings in order to secure the best outcomes and safe discharges for the Halton residents you will be working with.
