Carers FIRST is leading carer’s charity providing information, advice and support to young people and adults who look after a relative or friend who could not manage without their help. We currently serve carers in seven areas in the South East, London and East of England and are recruiting two part-time Carers Health & Wellbeing Coordinators to work within seven primary care networks in Medway. You will work alongside the Carer Health & Wellbeing Team Lead to develop the support for carers within GP surgeries
This is a new role working within the 7 Primary Care Networks in Medway. The role will develop the support for carers within GP surgeries. You will be responsible for raising the awareness of carers within the surgeries among all the staff team. You will support the surgeries to keep the carers list up to date and offer additional services to carers such as health checks. You would be responsible for the promotion of Carers FIRST and the services they offer to carers of all ages. This role will support GP practices to achieve the quality markers for supporting carers promoted by NHS England.
You will be self-motivated and have a positive approach to working with carers. You need to be able to respond promptly to new challenges and be influential in supporting carers to manage their lives to improve their health and well-being. You will be managing your own diary so will need to have good time management skills and good IT skills.
Your hours will be 22 hours per week and may include some unsocial hours to meet the needs of the surgeries and carers.
You need to have a full driving licence and access to a car. You will be required to travel between the different surgeries within the PCN and to undertake home visits on occasions.
