The YC Hertfordshire LDD team provides a range of specialist services for young people (aged 14-25) with learning difficulties and/or disabilities (LDD) and those who are Leaving Care. This involves providing information, advice and guidance to support them in making positive changes in their lives.
We have successfully secured funding for a one year ‘My Future’ Pilot Programme, starting in September 2020. This programme has been designed to support young people with Education, Health and Care Plans (EHCPs) to transition out of college into a positive destination or opportunity.
We are currently looking to recruit a full time ‘My Future’ Coach to work on the programme. You will be part of the YC Hertfordshire LDD Team and your office base will be in Stevenage, at Farnham House. The key role of the ‘My Future’ Coach is to support the delivery of the ‘My Future’ Transition Plan and provide transition coaching assistance to young people with learning difficulties and/or disabilities, working closely with parents/carers.
You will work within a small programme team, will need to have experience in working with young people with LDD and have some understanding of the barriers to learning, training and employment that young people with LDD may have.
You will need to be able to travel independently around the county so a driving licence and access to a vehicle is essential.
