About the role
This interesting post is within the Hospital Discharge Team. The team covers a number of hospitals within our local area. The post offers the opportunity to work in partnership with adults, their families and carers, to enable them to identify the support and outcomes which they would like to achieve.
The post would offer the preferred candidate the opportunity to work in partnership with our clients, undertaking Care Act (2014) compliant assessments, offering support, guidance and signposting where necessary.
You will need to be confident and competent in assessing risk, undertaking Care Act compliant assessments of an individual’s care and support needs, mental capacity assessments and to have knowledge of Safeguarding Adults and be willing to develop in these areas.
A commitment to your own professional development is also required with good support and encouragement.

Your starting salary will be pro rata if the above position is part-time or term-time only.
Work Permits: we may be able to obtain a Work Permit for this post but this is subject to meeting the requirements of the UK Border Agency’s Points-based Immigration System.
