The Transitions Service support young people from the ages of 16-25 with severe and enduring disabilities to move into adult services in a planned and personalised way. We are looking for a motivated and proactive Social Worker/Resource Officer who is committed to ensuring that the service supports young people and their families to make choices about their future that are individual, timely and appropriate to join our team.
To fully support young people, the service sits within Adult Social Care. However, our work necessitates close working partnerships with Children’s Disability Social Care, Education and Health Services.
As a service we are required to work within the Legislative Acts and Frameworks covering both children and adults. You will need to have knowledge and an understanding in this area of practice. Recent experience of children’s and adults social work, supporting young people with disabilities and their families, and a thorough awareness of relevant legislation relating to children and adults with disabilities would be advantageous.
