Safe & Settled Co-ordinator (Hospital Discharges)

We are looking for a highly motivated, organised and enthusiastic person to lead a hospital discharge support service. Safe & Settled is a new service that will be based at the Royal Surrey County Hospital and will be managed by the Guildford & Waverley Care & Repair Team. The aim of the service is to work alongside hospital staff to offer support to older and vulnerable residents living in Guildford & Waverley, helping to facilitate timely and safe discharges.  After a period of support and when the patient has settling back home the Safe & Settled team will link residents to community services where appropriate to enhance wellbeing and reduce loneliness and isolation.

Job Profile

What will you be doing on a typical day

  • Establish policies and procedures that underpin the implementation of the Safe & Settled service with the Care & Repair Team Leader
  • Promote Safe & Settled across the health system
  •  Liaise with the various hospital departments to identify suitable patients that would benefit from Safe & Settled 
  • Attend daily multi-disciplinary discharge meetings across the system 
  • Work with health care professionals to prioritise Safe & Settled patients 
  • Liaise with hospital transport services 
  • Onward referrals to Guildford & Waverley Borough council services 
  • Referrals to voluntary sector 
  • Monitoring and recording intervention 
  • Networking with social care team, voluntary sector, Guildford & Waverley borough council and wide range of departments within the RSCH  
  • Schedule visits for the Safe & settled support officer 
  • To coordinate wellbeing telephone calls 
  • Review wellbeing calls and prioritise levels to high/moderate/low risk. Take appropriate action if a patient feels unwell, not eaten or taken medication etc 
  • Review cases and sign off patients after intervention 
  • Supervision of support officer 
  • Knowledge and understanding of Surrey County Council Safe guarding procedure and the skill to identify vulnerable adults 
  • Responsible for accurate recording of Safe & Settled outcomes  

 

At A Minimum you will

Have a degree in Occupational Therapy, Social work or Diploma in Health & Social Care or equivalent with extensive experience within health, Social Care or voluntary sector.

  • Skills to complete assessment of need
  • Sound reasoning skills to achieve desired outcomes
  • Contribute to Multi-disciplinary team meeting
  • To challenge risk adverse behaviours
  • Sound knowledge of presentation and impact of a wide range of medical conditions
  • Knowledge of equipment available to support safe discharge and enhance independence
  • Knowledge to identify Safeguarding situations and take the appropriate action to minimise risk to patients.
  • Be able to guide and advise support worker when a patient post discharge presentation is deteriorating and potentially may need medical intervention.
  • Being highly mobile it is essential to be able to work independently and use own initiative and have sound reasoning skills when planning discharge An enhanced DBS disclosure and a Children and Adults barred list check will be required for this role
  • Car driver and access to own vehicle essential
