Job Title: Team Manager, Integrated Discharge Team
Location: Watford General Hospital
Salary Range: £45,591 - £48,531 pa + £4000 Market Forces pa
Hours: 37 per week
An opportunity has arisen for a Team Manager post in the Watford General Hospital Integrated Discharge Team. As a qualified Social Worker, or Occupational Therapist with experience in management, you will be joining a strong and well-staffed team supporting people with complex needs returning home from hospital.
The Integrated Discharge Team (IDT) is made up of health and social care staff, as well as colleagues from private and voluntary care organisation all supporting timely discharge planning. As Team Manager you will be managing and developing the social care part of the service working alongside the Head and Deputy Head of Service to deliver integrated discharge planning and pathways. This will include for example the ongoing development of 7 day working and Discharge Home to Assess pathways.
The successful candidate will join an IDT that has seen significant improvements in its delivery of the practice and the performance agenda in recent years. The Team has led on the Hertfordshire Connected Lives Assessment Framework delivering personalised and outcome focussed care planning upon discharge and in the community, as well as exceeding NHSE Targets for people delayed awaiting Social Care resource in the last 12 months.
In this role you will need to have highly developed communication and organisational skills, an ability to lead and motivate your staff group, provide assurance to system partners and work across organisational boundaries to influence, problem solve and support complex situations. You will need to demonstrate the creativity and the ability to develop services and pathways, as well as an understanding of hospital care pathways, community resources, The Care Act and Delayed Transfers of Care.
With the ability to monitor and report the impact and outcome of the service performance you will possess skills in budget management and reporting. You will also be a creative thinker who can work with community providers to come up with innovate solutions to complex problems.
