About the Post:
The Case Worker will be responsible for undertaking assessments of households in their own homes, working with residents and other stakeholders to implement actions to reduce fuel poverty, address cold homes and signpost on to help to improve housing conditions. This will involve case management of low level support provision to enable independence at home and facilitate hospital discharges where required.
This role will provide a triage function ensuring that referrals and pathways are appropriate to the presenting level of needs and risk, working in partnership with a range of organisations and groups across the borough in order to achieve the best possible outcomes for individuals, families and communities.
