About the role
This is a 12 month fixed term contract.
Surrey Domiciliary Care Service is part of the Short Breaks team, receiving referrals from the Children with Disabilities Team Social Workers.
We are a countywide service with bank support workers, providing short breaks for disabled children, young people and their families in their home and in the community.
As a service we have significant expertise in supporting children and young people with complex needs, including autistic spectrum conditions, challenging behaviour, profound and multiple learning disabilities and associated complex health needs.
The team is made up of the Service Manager, one full time and two part time Coordinators.
The role involves assessing children following a referral from the Social Worker. It requires direct involvement with disabled children and their families. The Coordinator will effectively match the child and family to an appropriate support worker within the agreed care plan for the child
The role demands a high level of partnership work with other professionals; notably Social Workers, education and health professionals.
The role holder will assist in the day to day runnings of the service and monitor its financial demands, as well as ensuring that the service meets the required essential standards of the Care Quality Commission.
To be shortlisted to interview for this position your application will clearly evidence:
- Diploma health and Social care Level 3 (Previously NVQ3), Diploma Children and young Peoples workforce (Previously NVQ3) or equivalent and/ or a commitment to work towards this.
- Experience of working with disabled children, demonstrating a positive approach to a social model of disability.
- Some experience of leading and developing staff
- Ability to work under pressure and be decisive
- Flexible attitude to working hours and the ability to travel around the county is essential.
