We are supporting social workers, occupational therapists and case managers to achieve excellent outcomes for our residents who are in need of advice, early intervention or longer term care and support. We have Senior Practitioner, Social Work (including NQSW), Case Manager, Support Worker, and O.T roles available in the following Teams:
Locality Teams – working as part of a growing multi-disciplinary team undertaking strengths based assessments, person centred support planning, reviews, safeguarding
Urgent Response – working with police, community health staff and individuals and families to provide a person centered urgent response, undertaking strength based assessments, initial safeguarding enquiries and providing advice, information and guidance.
Hospital Team – Working closely with ward staff, re-ablement and the discharge to assess team carrying our strength based assessments, safeguarding enquiries and direct work with individuals and families to ensure a safe discharge home.
Integrated Learning Disability Team – working as part of a large integrated learning disabilities service carrying out strength based social work assessments and reviews, person centered support planning and leading safeguarding enquiries.
Mental Health Teams –Staff are seconded into the Mental Health Trust and work as part of a multi-disciplinary team providing strengths based social work assessments and reviews, leading safeguarding enquiries and direct work with individuals and their families.
Emergency Duty Team – undertake urgent assessments and safeguarding out of hours, including weekends
Re-ablement Service – working as part of a multi-disciplinary team to support positive outcomes form re-ablement undertaking strength based assessments, goal planning, direct support and review to enable people to continue living in the community
Occupational Therapy Team – assessing people and their home environment using strengths based approaches, providing advice, equipment and adaptations.
Sensory Team – Working as part of a multi-disciplinary sensory service undertaking strengths based social work assessments and reviews, person centered support planning and working with the team manager to develop the work of the service.
Community and Placement Review Team reviewing community and residential packages of care ensuring the customers are getting care appropriate to their needs. Undertaking complex safeguarding work ensuring well-being and safety of our customers is paramount
