An exciting opportunity has arisen to be part a new Social Prescribing team within Public Health, supporting Medway’s older population to become less socially isolated.
The postholder will contribute to the development and implementation of the Social Prescribing Programme in Medway by asset mapping the services that are available within the community for link workers to prescribe to. Identifying any gaps and mobilising the community sector to provide an initiative. You’ll work collaboratively with all local partners, to ensure that local VCSE organisations and community groups are sustainable and that community assets are nurtured. Researching avenues of funding and making them aware of small grants or micro-commissioning if appropriate.
You’ll support the delivery and roll out of a directory of services that will be accessed by numerous stakeholders across Medway. You’ll use this system to collate and analyse a wide range of complex quantitative and qualitative information to identify the health needs of the local population and translate this into an accessible format to enable the identification of priorities and inform the development of local services and interventions.
An important part of the role will be designing and delivering training to a wide range of stakeholders, to ensure appropriate referrals in to the service and promote the social prescribing agenda across Medway.
Additional information - You may be required to work evenings and weekends.
Successful candidates must:
- Be passionate and committed to improving the lives of Medway’s socially isolated older population
- Have excellent influencing, communication and interpersonal skills
- Be a confident public speaker
- Have experience of inter-agency working with a broad range of statutory organisations, external agencies, community and voluntary organisations
- Have experience of developing effective partnerships to facilitate the development and implementation of health improvement programmes or other inter-agency interventions
- Represent Public Health and champion the social prescribing agenda
- Be able to work independently and make decisions without access to a manager
- Hold a full driving license – non negotiable
