In Northampton, we take rough sleeping very seriously.
We have expanded our Single Homelessness Pathway Service to include three Housing Transitions Officers who will prevent homelessness by providing an advice, support and resettlement service for people who are being discharged from hospital, being released from prison or are leaving local authority care.
Our Housing Transitions Officer (Hospitals) will work closely with Northampton General Hospital, Berrywood Hospital, the Street Outreach Workers and Rough Sleeper Navigators, and local housing providers to ensure that patients’ discharge from hospital is carefully planned and they receive appropriate help and support.
The successful candidate will carry out a comprehensive assessment of patients’ needs and options and work collaboratively and proactively with health and social care professionals to encourage an innovative, solution-focused, multi-agency approach to ensuring patients’ smooth transition into appropriate, settled housing.
To meet the requirements of this exciting role, you will need to be an excellent team player, an adept problem solver and have a proven track record of building positive, trusting relationships with a wide range of partners and stakeholders.
You will also need to be very well organised, have excellent casework management skills, be able to work under pressure and on your own initiative, and have experience of working with people who are homeless and/or have complex needs.
