We are looking for a full time Social Care Practitioner (with an Occupational Therapy qualification).
You will be based at Royal Surrey County Hospital and would work at the 'front door' (Accident and Emergency, Frailty Short Stay Unit and Emergency Assessment Unit).
£34,688 - £38,476 per annum for 36 hours per week (full time) based at Royal Surrey County Hospital, Egerton Road, Guildford, GU2 7XX.

About the role
We are seeking a qualified, experienced professional to enable a holistic assessment of needs to be undertaken, including functional assessments and social care assessments to enable a fast and safe discharge from the hospital environment, to prevent hospital admission.
Our core function is to facilitate patient flow by supporting discharges to the destination of choice in a timely manner for social care and privately funded patients through assessments and liaising with care providers.
You will need to be motivated and flexible with high professional standards and sound knowledge and experience of current legislation, including the Care Act, Mental Capacity Act and Continuing Health Care framework and its application to practice in a social care/hospital setting. You will be able to communicate in a clear, fluent, concise and jargon-free way and in a courteous, calm and professional manner.
You will need to be confident in undertaking safeguarding enquiries, carrying out assessments within a busy, fast-paced hospital setting and working with a diverse group of patients, their families and partner agencies.
You will have experience of working within Adult Social Care and with various client groups i.e. older adults, physical disability, learning disability.
