The role of Transfer of Care Social Worker is a new post, sitting within the community mental health Assessment and Treatment Service. The successful candidate will be supported by the local team managers and service managers to develop the post. In particular you will focus on finding and implementing solutions to blockages in care being delivered in a timely fashion to individuals.
Are you a highly motivated, experienced senior mental health social worker who is able to engage quickly and effectively with clients to help them to understand their needs and actively plan their care and support?
Are you skilled in engaging clients, carers and staff to identify the correct level of support which will allow people who are inpatients in acute hospital settings and rehabilitation to continue their recovery back in the community?
This role requires excellent communication skills and someone who is able to adapt, with a flexible and proactive approach to their work. You will be required to work autonomously while at the same time developing strong working relationships with colleagues in each of the community teams so that planned care is delivered safely and effectively.

This is a senior practitioner social worker post and will also carry responsibility for supervising senior social workers and supporting the teams more widely as well as a playing a role within the service development across North West Sussex area.
The post covers all areas of North West Sussex and will provide a service to all clients whose care is provided by Crawley ATS, Horsham ATS and Mid Sussex ATS.
You will also be an AMHP. The post sits in the S75 secondment to Sussex Partnership NHS Trust.
