If so, we want you to join our Short Term Assessment and Reablement Team in Amber Valley area of Derbyshire.As a Social Care Practitioner, you will play a vital role in supporting adults to regain independence following illness, injury, or a change in circumstances. Working closely with individuals, families, and multi-disciplinary professionals, you will assess needs, develop person-centred plans, and provide short-term interventions that empower people to live safely and confidently in their own homes.

What You’ll Do

  • Carry out holistic assessments and reviews in line with Care Act principles.
  • Work collaboratively with health professionals, therapists, and community services.
  • Promote independence and wellbeing through tailored reablement plans.
  • Ensure safeguarding and risk management are at the heart of your practice.
  • Use strength-based approaches to achieve positive outcomes.

About You

  • Experience in adult social care or a related field.
  • Strong communication and problem-solving skills.
  • Ability to work flexibly and manage a varied caseload.
  • Commitment to promoting dignity, choice, and independence.
  • Knowledge of relevant legislation and best practice.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
