You will need to show excellent communication skills and be able to provide service users and carers with information, advice, guidance and support and enable them to create effective Support Plans which will meet their identified care needs
The post holder will have a good knowledge of additional sources of help within the community, private sector and social care networks which may be accessible to individual service users.
Interview date: Tuesday 30th September 2010.
