An opportunity has arisen for Registered Social Workers in a project team to review people in receipt of community based care/support, and a long term area based team in assessment and care management.
The post holder will need to work closely with other members of the team and other professionals both in Social Care and Health in order to deliver a professional assessment service to support a programme of fair access to care reviews.
The successful applicant will have an excellent understanding and working knowledge of social care legislation including fair access to care services and the Council’s eligibility criteria. They will need to possess excellent assessment and risk management skills, with a thorough knowledge of safeguarding and the protection of vulnerable adults community care legislation and Mental Health legislation including the mental capacity act
This is a 6 month fixed term contract

The post requires the post holder to hold a full drivers licence.
