We are looking for an experienced Care Manager with a Social Work, Nursing or Occupational Therapy qualification to join our busy Health and Social Care team as a Senior Care Manager.
You will be working with a diverse range of service users with a generic older people caseload, as well as younger people with physical and sensory disabilities.
You will be expected to work with complex cases, carrying out assessments, care planning, reviews and assisting with duty cover. You must have a sound knowledge of safeguarding.
There will be a supervisory capacity within the role with Community Support Workers, Advisory Officers, or Care Managers within the team.
An enhanced Criminal Records Bureau disclosure and POVA check will be required for this role.
