Location Based in Weston-super-Mare

We are seeking a Reviewing Officer to work in the Personalisation Team based in Weston-super-Mare. This team works with adults who are experiencing problems relating to ageing, physical/sensory impairment and other serious health and social care problems

Duties include reviewing care plans and care arrangements as set up by Care Managers and making any changes as required in consultation with the individual; their carers and other professionals as appropriate.

The Reviewing Officer will be expected to thoroughly review the services people are receiving and check they are meeting eligible needs in a cost effective way. The officer will also encourage and enable people who use social care services to take up the choices and control offered by the Personalisation Agenda.

Experience of working within a social care setting and with the care management process would be desirable as would knowledge of Fair Access to Care Services (FACS)

The team is working to promote all aspects of personalisation and is committed to multidisciplinary working with locality adult care teams, health, housing, voluntary and private agencies
