The Assessment and Care Management Service is about to undergo significant transformational change.

This post is required as part of our change programme. We have a clear vision of where we need to be to enable the service to deliver a modern flexible service to customers. Key elements of this vision are;

mainstreaming self directed support
development of self assessment and self service to improve choice and control
re-engineering services to promote prevention and early intervention
Strengthening advocacy and brokerage facilities
Increasing access to enablement services
Maintaining effective safeguarding arrangements

You will lead staff, partners and customers through the changes for the future, including the development of effective systems and processes whilst maintaining high quality services.

You will work closely with the Interim Head of Vulnerable Adults to drive forward the modernisation of the service.

You will have specific responsibility for leading the Enablement service, which is a fast paced service which provides outcome focused support to people.

To succeed in this post you will;

Be enthusiastic, creative and able to make the vision a reality
Demonstrate a track record in multi agency working
Have strong leadership and management skills
Have a proven track record in managing change and business transformation to deliver measurable outcomes for people.

You will be required to register with the General Social Care Council, and the Independent Safeguarding Authority in line with the relevant timescales.

Satisfactory disclosure at the appropriate level under the Criminal Records Bureau is required prior to appointment.

An assessment centre will take place for short listed applicants.
