Based in The Carer Services Team you will work as part of a multi disciplinary team, working in partnership with NHS Bury, GP practices, hospitals and other local organsiations. You will work towards the objectives set within the Carers Strategy identifying and supporting new and existing informal Carers within Health. This role involves liaising with all GP practices in the Bury Borough to identify, providing support, advice and information to patients who are Carers. You must be enthusiastic with a knowledge if Carers’ issues.
Hours of work are 25 per week.
This post is grant funded and is currently scheduled to operate until 31 March 2010.
Satisfactory disclosure at the appropriate level under the Criminal Records Bureau is required prior to appointment.
