Location Joynes House Gravesend

The Dartford, Gravesham and Swanley Coordination Team, based in Gravesend are seeking to recruit a Case Manager (OT) to join their busy team enabling adults with physical disabilities, frailty or dementia to live as full a life as possible in the community.

This is a new role to promote enablement and independent living within the Care Management team, working alongside Case Managers, developing support plans and reviewing complex care packages aiming to reduce dependency on care services.

We are looking for an experienced, registered Occupational Therapist, with sound assessment skills and experience of enablement/rehabilitation. You will need a proven ability to manage a complex caseload and be a good team player.

Flexible and mobile working is encouraged. We offer supervision, support, career development and comprehensive training.
