We have a vacancy for a Case Manager to work in the Kent & Canterbury Hospital, supporting safe and timely discharges for clients who are admitted into the acute trust. We are looking for a confident and competent team member, who can work autonomously as well as part of the multidisciplinary team supporting service users and their carers in planning their future care needs.
You will bring with you excellent assessment skills and knowledge of Enablement and safeguarding which will be essential in your role. You will work closely with health colleagues and other providers to maximise wellbeing and choice for service users.
We provide an induction programme and ongoing training and development is available and actively encouraged.
